Ecological Falls Data Collection Tool

Falls are a common concern that school based PTs can involved in.  Often a teacher says" the student falls alot" but presents to specifics or data, just a subjective response when asked.  This tool has the classroom staff look at antecedents, the actual fall, the student response, peer response and classroom staff response in order to tease out the true cause of the concern.
